Patient Usually Complains Of
Patient presents with subacute progressive renal insufficiency following [recent vascular procedure/anticoagulation initiation]. Reports associated symptoms including livedo reticularis, blue toe syndrome, and transient abdominal pain. Denies recent nephrotoxic exposure or obstructive uropathy.
Clinical Examination Findings
General appearance: Patient in no acute distress. Skin: Note presence of livedo reticularis on lower extremities, cyanotic digits (blue toe syndrome), and palpable purpura. Vitals: Hypertension noted. Peripheral pulses: Asymmetric or diminished pulses in lower extremities.
Treatment Protocol
Management plan: Discontinuation of anticoagulants/thrombolytics. Strict blood pressure control using ACE inhibitors or ARBs. Initiation of high-intensity statin therapy. Avoidance of further vascular instrumentation. Monitor renal function and inflammatory markers. Consider surgical consultation for aortic plaque stabilization if indicated.
