Patient Usually Complains Of
Patient presents with acute onset of hematemesis and/or melena. No prior history of peptic ulcer disease, NSAID use, or alcohol abuse. Hemodynamic status is [stable/unstable]. Symptoms characterized by sudden, painless, large-volume upper gastrointestinal bleeding.
Clinical Examination Findings
Abdominal examination reveals soft, non-distended, non-tender abdomen. No palpable masses or organomegaly. Bowel sounds are [normoactive/hyperactive]. Rectal exam confirms melena or hematochezia. Vital signs indicate [tachycardia/hypotension] consistent with acute blood loss.
Treatment Protocol
Urgent esophagogastroduodenoscopy (EGD) performed. Identification of a prominent, pulsatile submucosal vessel in the proximal stomach without overlying ulceration. Hemostasis achieved via [endoclip application/thermal coagulation/epinephrine injection]. Post-procedure management includes PPI infusion and monitoring for rebleeding.
