Clinical Presentation & Protocol

Patient Usually Complains Of

Patient presents with chronic retrosternal burning sensation (heartburn) and acid regurgitation, occurring [frequency] times per week. Symptoms are exacerbated by recumbency, postprandial state, and consumption of [trigger foods]. Denies dysphagia, odynophagia, unintentional weight loss, or hematemesis. Current symptom severity score: [0-10].

Clinical Examination Findings

General: Patient is in no acute distress. HEENT: Oropharynx shows no erythema or cobblestoning. Cardiovascular: Regular rate and rhythm, no murmurs. Respiratory: Lungs clear to auscultation bilaterally. Abdomen: Soft, non-distended, non-tender to palpation, no guarding or rebound. Epigastric region non-tender. Bowel sounds present in all four quadrants.

Treatment Protocol

Initiate lifestyle modifications: elevate head of bed, avoid late-night meals, and eliminate dietary triggers (caffeine, alcohol, spicy foods). Pharmacotherapy: Proton Pump Inhibitor (PPI) [Drug Name] [Dose] once daily, 30 minutes before the first meal. Follow-up in [Timeframe] to assess symptom control and potential for step-down therapy.
