Gaucher Disease with Bone Crisis

Clinical Presentation & Protocol

Patient Usually Complains Of

Patient presents with acute, severe localized bone pain consistent with a Gaucher bone crisis. Pain is described as deep, throbbing, and refractory to standard analgesics. Associated with localized tenderness, warmth, and limited range of motion in the affected limb. No history of recent trauma. Known history of Gaucher disease (ICD-10: E75.22) with prior episodes of osteonecrosis or bone infarction.

Clinical Examination Findings

General: Patient appears in acute distress due to pain. Musculoskeletal: Localized bony tenderness, swelling, and erythema noted over the affected site (e.g., femur/humerus). Decreased range of motion due to pain. Neurovascular: Distal pulses intact, capillary refill <2 seconds, no sensory or motor deficits. Abdominal: Palpable hepatosplenomegaly noted, consistent with underlying Gaucher disease progression.

Treatment Protocol

1. Aggressive pain management: Initiate IV opioids and non-steroidal anti-inflammatory drugs (NSAIDs) as indicated. 2. Hydration: Maintain adequate fluid intake. 3. Immobilization: Rest the affected limb to prevent pathological fracture. 4. Disease-specific therapy: Review current Enzyme Replacement Therapy (ERT) or Substrate Reduction Therapy (SRT) dosing; consider escalation if indicated. 5. Orthopedic consultation: Evaluate for osteonecrosis or impending fracture.
