Patient Usually Complains Of
Patient presents for evaluation of congenital vascular malformation triad: capillary malformation (port-wine stain), venous varicosities, and limb hypertrophy. Onset noted at [Age/Birth]. Current symptoms include [pain/heaviness/recurrent cellulitis/bleeding]. Review of systems negative for deep vein thrombosis (DVT) or pulmonary embolism (PE) symptoms.
Clinical Examination Findings
Physical exam reveals: 1. Cutaneous: Geographic capillary malformation involving [Location]. 2. Vascular: Prominent, dilated, tortuous superficial veins in [Location], non-compressible. 3. Musculoskeletal: Limb length discrepancy of [X] cm; girth discrepancy of [X] cm. Distal pulses [present/absent]. Skin temperature [warm/cool]. No signs of acute infection or ulceration.
Treatment Protocol
Management plan: 1. Compression therapy: Custom-fitted medical-grade compression garments. 2. Symptomatic: Elevation of affected limb, analgesics as needed. 3. Interventional: Referral to Interventional Radiology for sclerotherapy or endovenous ablation of symptomatic varicosities. 4. Orthopedic: Shoe lift for limb length discrepancy. 5. Surveillance: Regular monitoring for DVT and coagulopathy.
