Clinical Presentation & Protocol
Patient Usually Complains Of
Patient presents with symptoms consistent with critical limb ischemia (CLI), including [rest pain/non-healing ulcer/gangrene] of the [left/right] [lower extremity/foot]. Pain is described as [constant/burning/aching], exacerbated by elevation and relieved by dependency. Duration of symptoms: [number] weeks. Associated symptoms include [claudication/paresthesia/coldness]. History significant for [smoking/diabetes/hypertension/hyperlipidemia].
Clinical Examination Findings
Physical exam reveals [absent/diminished] pedal pulses (dorsalis pedis/posterior tibial). Skin shows [pallor/dependent rubor/cyanosis] with [atrophic changes/loss of hair/thickened nails]. Capillary refill time is >[3] seconds. Ulceration noted at [location], measuring [size] cm, with [necrotic/granulation] tissue base. Temperature is [cool/cold] compared to contralateral limb. ABI (Ankle-Brachial Index) measured at [value].
Treatment Protocol
Immediate management plan: 1. Initiation of antiplatelet therapy [Aspirin/Clopidogrel]. 2. Statin therapy for plaque stabilization. 3. Optimization of glycemic control and blood pressure. 4. Wound care consultation for local debridement. 5. Urgent vascular surgery/interventional radiology referral for revascularization (angioplasty/stenting/bypass). 6. Smoking cessation counseling.
