Patient Usually Complains Of
Patient presents for evaluation of suspected PCOS. Reports chronic oligomenorrhea/amenorrhea, symptoms of hyperandrogenism including hirsutism (Ferriman-Gallwey score: [score]), acne, and/or androgenic alopecia. Denies galactorrhea, thyroid symptoms, or rapid weight gain. Family history significant for metabolic syndrome or PCOS.
Clinical Examination Findings
Vitals: BMI [value] kg/mยฒ. General: No signs of Cushingโs syndrome or virilization. Skin: Presence of acanthosis nigricans in skin folds, moderate facial/chest acne. Hirsutism: Terminal hair growth noted in androgen-dependent areas. Pelvic Exam: Normal external genitalia; bimanual exam reveals no adnexal masses or significant tenderness.
Treatment Protocol
Plan: 1. Lifestyle modification (weight loss, low-glycemic diet). 2. Combined Oral Contraceptives (COCs) for cycle regulation and androgen suppression. 3. Metformin [dosage] for insulin resistance. 4. Spironolactone [dosage] for hirsutism. 5. Follow-up in [timeframe] to monitor metabolic profile and treatment response.
