Clinical Presentation & Protocol
Patient Usually Complains Of
Patient presents with chronic, progressive right knee pain, stiffness, and swelling. Symptoms are worse in the morning, lasting >60 minutes. Reports associated joint warmth and functional limitation in ambulation. No history of trauma. Systemic symptoms include fatigue and morning stiffness.
Clinical Examination Findings
Right knee examination reveals synovial thickening, palpable effusion, and tenderness along the joint line. Range of motion is limited by pain and stiffness. No ligamentous laxity. Signs of inflammation (erythema/warmth) present. Gait analysis shows antalgic pattern favoring the left side.
Treatment Protocol
Initiate DMARD therapy as per rheumatology protocol. Prescribe NSAIDs for acute pain management. Recommend physical therapy for range of motion and quadriceps strengthening. Consider intra-articular corticosteroid injection if effusion persists. Monitor inflammatory markers (ESR/CRP) and liver/renal function.
