Clinical Presentation & Protocol

Patient Usually Complains Of

Patient presents with ulnar-sided wrist pain exacerbated by rotational movements (pronation/supination) and ulnar deviation. Reports a history of [acute trauma/repetitive strain] involving the wrist. Pain is localized to the distal radioulnar joint (DRUJ) and the ulnocarpal space. Denies numbness or paresthesia in the ulnar nerve distribution.

Clinical Examination Findings

Physical examination reveals localized tenderness at the fovea and the ulnar styloid. Positive ulnocarpal stress test. TFCC grind test elicits pain and/or crepitus. DRUJ stability assessment demonstrates [stable/lax] joint play compared to the contralateral side. Grip strength is diminished due to pain. No evidence of distal neurovascular compromise.

Treatment Protocol

Recommended treatment plan: Immobilization with a wrist splint for 4-6 weeks to facilitate healing. Initiation of physical therapy focusing on range of motion and strengthening of the forearm stabilizers. NSAIDs for pain and inflammation management. If symptoms persist, consider MRI imaging to evaluate for Palmer classification tear type and potential surgical intervention (arthroscopic debridement or repair).
