Cryoglobulinemic Glomerulonephritis (Mixed, Type II)

Type II cryoglobulinemia consists of polyclonal IgG and monoclonal IgM (often with rheumatoid factor activity). Highly associated with Hepatitis C infection. Renal biopsy shows MPGN pattern with intraluminal cryoglobulin 'thrombi'.

Clinical Presentation & Protocol

Patient Usually Complains Of

Patient presents with clinical features of mixed cryoglobulinemic vasculitis, including palpable purpura, arthralgias, and progressive renal insufficiency. History is significant for [HCV status], with recent onset of hematuria, proteinuria, and hypertension. No history of systemic lupus or other connective tissue disorders.

Clinical Examination Findings

General: Patient appears chronically ill. Skin: Palpable purpura noted on lower extremities, non-blanching. Joints: Mild tenderness and swelling in small joints of hands. Vitals: Hypertension noted (BP: [Value]). Edema: 1-2+ pitting edema noted in bilateral lower extremities.

Treatment Protocol

Management plan: 1. Immunosuppression: [e.g., Rituximab/Cyclophosphamide/Corticosteroids] initiated. 2. Antiviral therapy: HCV-directed therapy as per hepatology consultation. 3. Plasma exchange: Indicated for severe organ-threatening disease. 4. Supportive: ACE inhibitors/ARBs for proteinuria management and BP control.
