Clinical Presentation & Protocol
Patient Usually Complains Of
Patient presents with acute onset of right elbow pain and weakness following a sudden eccentric load or "pop" sensation. Reports significant bruising in the antecubital fossa and difficulty with supination and elbow flexion. No prior history of similar symptoms.
Clinical Examination Findings
Right upper extremity exam reveals ecchymosis and swelling in the antecubital fossa. Palpation demonstrates a palpable defect at the distal biceps insertion. Hook test is positive, indicating inability to hook the biceps tendon. Active flexion and supination strength are significantly diminished compared to the contralateral side. Neurovascular status is intact distally.
Treatment Protocol
Immobilization in a posterior splint at 90 degrees of flexion to minimize tension. Referral to orthopedic surgery for consideration of primary surgical repair. Analgesics prescribed for pain management. Advised to avoid lifting or strenuous activity with the affected arm.
