Clinical Presentation & Protocol

Patient Usually Complains Of

Patient presents for evaluation of a hepatic lesion identified on imaging. History significant for [oral contraceptive use/anabolic steroid use/metabolic syndrome]. Patient reports [asymptomatic/RUQ abdominal pain/epigastric discomfort]. No history of cirrhosis, chronic hepatitis B/C, or elevated alpha-fetoprotein. Denies constitutional symptoms such as weight loss, night sweats, or fever.

Clinical Examination Findings

Abdominal examination reveals [soft/distended] abdomen. No palpable hepatomegaly or splenomegaly. No evidence of ascites or caput medusae. Bowel sounds present and normal. No localized tenderness on deep palpation of the right upper quadrant. Skin examination negative for stigmata of chronic liver disease (e.g., spider angiomata, palmar erythema).

Treatment Protocol

Management plan: 1. Discontinuation of offending agents (e.g., oral contraceptives). 2. Weight management and metabolic optimization if associated with NASH. 3. Serial cross-sectional imaging (MRI with hepatobiliary contrast) every 6 months to monitor lesion size and stability. 4. Surgical consultation for resection if lesion >5cm, symptomatic, or showing signs of hemorrhage/malignant transformation.
