Patient Usually Complains Of
Patient presents with symptoms suggestive of hypothyroidism, including fatigue, weight gain, cold intolerance, constipation, and dry skin. Duration of symptoms is [insert duration]. Review of systems is positive for [e.g., bradycardia, hair loss, menorrhagia, or cognitive slowing]. No history of neck radiation or thyroid surgery. Family history significant for autoimmune thyroid disease.
Clinical Examination Findings
General: Patient appears [e.g., lethargic/well-nourished]. HEENT: Thyroid gland is [e.g., non-palpable/diffusely enlarged/firm/nodular]. No thyromegaly or tenderness noted. Cardiovascular: Regular rate and rhythm, bradycardia noted at [X] bpm. Skin: Dry, coarse texture, non-pitting edema noted in [e.g., lower extremities]. Neurological: Delayed relaxation phase of deep tendon reflexes (Woltman sign).
Treatment Protocol
Initiate Levothyroxine therapy at [X] mcg daily, taken on an empty stomach 30-60 minutes before breakfast. Advise patient to avoid calcium or iron supplements within 4 hours of medication. Schedule TSH/Free T4 monitoring in 6-8 weeks for dose titration.
