Patient Usually Complains Of
Patient presents with chronic anterior right knee pain localized to the inferior pole of the patella. Symptoms exacerbated by repetitive loading activities, specifically jumping, running, or prolonged sitting. Pain is described as aching and sharp during activity, with morning stiffness. No history of acute trauma, locking, or instability.
Clinical Examination Findings
Right knee examination reveals localized tenderness at the inferior pole of the patella and the patellar tendon insertion. No significant effusion or erythema noted. Range of motion is full and painless. Resisted knee extension elicits pain at the patellar tendon. Negative Lachman, McMurray, and varus/valgus stress tests. Patellar tracking is normal.
Treatment Protocol
Initiate conservative management including activity modification, relative rest, and ice application post-activity. Prescribe physical therapy focusing on eccentric strengthening exercises for the quadriceps and patellar tendon. Consider patellar tendon strap for symptom relief during activity. NSAIDs as needed for pain management.
