Acute inflammatory arthritis caused by calcium pyrophosphate crystal deposition in the right knee.

Clinical Presentation & Protocol

Patient Usually Complains Of

Patient presents with acute onset of severe right knee pain, swelling, and erythema, starting [Timeframe]. Reports significant limitation in range of motion and difficulty with weight-bearing. Denies recent trauma or fever. History significant for known CPPD arthropathy.

Clinical Examination Findings

Right knee examination reveals marked effusion, warmth, and diffuse tenderness along the joint line. Erythema noted over the suprapatellar bursa. Range of motion is restricted by pain, specifically in flexion. No signs of cellulitis or skin breakdown. Neurovascular status intact distally.

Treatment Protocol

Plan: 1. Arthrocentesis of the right knee for synovial fluid analysis (cell count, crystals, culture). 2. Intra-articular corticosteroid injection post-aspiration. 3. Initiate NSAIDs (e.g., Naproxen or Indomethacin) or Colchicine as indicated. 4. RICE protocol (Rest, Ice, Compression, Elevation). 5. Follow-up in 1 week.
