EN: Continuous daily oral dosing of cyclophosphamide (typically 1.5 - 2.0 mg/kg/day) for induction of severe ANCA vasculitis. Requires meticulous monitoring for leukopenia, hemorrhagic cystitis, and opportunistic infections.
