Clinical Presentation & Protocol
Patient Usually Complains Of
Patient presents with a history of poorly controlled asthma, reporting increased frequency of wheezing, productive cough with brownish mucus plugs, and intermittent low-grade fevers. Symptoms are refractory to standard inhaled corticosteroid and bronchodilator therapy. No reported hemoptysis or night sweats.
Clinical Examination Findings
Chest auscultation reveals diffuse bilateral expiratory wheezing and occasional coarse crackles. No signs of digital clubbing or peripheral edema. Skin examination negative for rashes. Vital signs stable, oxygen saturation within normal limits on room air.
Treatment Protocol
Initiate systemic corticosteroid therapy (prednisolone) to suppress the inflammatory response. Consider adjunctive antifungal therapy (itraconazole or voriconazole) to reduce fungal burden. Monitor total serum IgE levels and eosinophil counts periodically. Ensure adherence to maintenance inhaled corticosteroids.
