Clinical Presentation & Protocol
Patient Usually Complains Of
Patient presents following high-energy trauma to the left lower extremity. Complains of severe left thigh pain, inability to bear weight, and visible deformity. Neurovascular status intact distally. No reported loss of consciousness or associated injuries.
Clinical Examination Findings
Left thigh demonstrates significant swelling, ecchymosis, and palpable bony crepitus at the mid-shaft. Shortening and external rotation deformity noted. Distal pulses (DP/PT) 2+ and symmetric. Capillary refill <2 seconds. Sensation intact to light touch in all dermatomes. No motor deficits noted in foot/ankle.
Treatment Protocol
Immediate immobilization with traction splint. Pain management initiated via IV analgesics. Orthopedic surgery consultation for intramedullary nailing. NPO status for potential urgent operative intervention. Serial neurovascular checks every 1 hour.
