Kienbock's Disease (Avascular Necrosis of Lunate), Left Wrist

ICD-10 Code
M87.132

Standardized diagnosis for Kienbock's Disease (Avascular Necrosis of Lunate), Left Wrist.

Clinical Presentation & Protocol

Patient Usually Complains Of

Patient presents with chronic, progressive left wrist pain localized to the dorsal aspect, exacerbated by gripping and weight-bearing activities. Reports associated stiffness, intermittent swelling, and decreased grip strength. No history of acute trauma. Symptoms are consistent with stage [Insert Stage] Kienbockโ€™s disease.

Clinical Examination Findings

Left wrist examination reveals localized tenderness over the lunate fossa. Range of motion is restricted in both flexion and extension. Grip strength is diminished compared to the contralateral side. No signs of distal neurovascular compromise. Scapholunate interval appears stable.

Treatment Protocol

Management plan includes immobilization with a wrist splint, activity modification, and NSAIDs for pain control. Referral for advanced imaging (MRI) to assess vascularity and staging. Surgical consultation for potential revascularization or unloading procedures (e.g., radial shortening osteotomy) pending disease progression.
