Clinical Presentation & Protocol

Patient Usually Complains Of

Patient presents with acute onset of pain and drooping of the left distal phalanx following a traumatic injury to the fingertip. Patient reports inability to actively extend the distal interphalangeal (DIP) joint. No history of prior injury to the digit.

Clinical Examination Findings

Physical examination of the left hand reveals localized tenderness and swelling over the dorsal aspect of the distal interphalangeal (DIP) joint. Active extension of the DIP joint is absent, with a persistent flexion deformity (lag). Passive extension is intact. Neurovascular status is intact with capillary refill < 2 seconds.

Treatment Protocol

Diagnosis of mallet finger confirmed. Treatment initiated with continuous immobilization of the left DIP joint in a neutral or slightly hyperextended position using a custom-molded splint. Patient instructed to maintain splinting 24/7 for 6-8 weeks. Avoidance of any flexion of the DIP joint during splint changes.
