Clinical Presentation & Protocol
Patient Usually Complains Of
Patient presents with persistent pain, decreased range of motion, and functional impairment of the right wrist following a prior distal radius fracture. Reports progressive stiffness, weakness in grip, and localized tenderness over the distal radius. No history of recent re-injury; symptoms attributed to symptomatic malunion.
Clinical Examination Findings
Right wrist examination reveals visible dorsal/volar angulation and radial shortening. Palpation demonstrates bony prominence at the distal radius with tenderness. Range of motion (ROM) is restricted in flexion, extension, and radial/ulnar deviation compared to the contralateral side. Neurovascular status: distal pulses intact, capillary refill <2 seconds, sensation intact in median, ulnar, and radial nerve distributions.
Treatment Protocol
Recommended management includes radiographic assessment (AP/Lateral/Oblique views) to quantify deformity. Options discussed: conservative management with physical therapy for functional optimization, or surgical intervention via corrective osteotomy with internal fixation (ORIF) to restore anatomical alignment and joint mechanics.
