Patient Usually Complains Of
Patient presents with acute onset of right knee pain following a twisting injury. Reports mechanical symptoms including locking, catching, and inability to fully extend the knee. Denies prior history of instability or chronic pain. Pain is localized to the medial joint line, exacerbated by weight-bearing and rotation.
Clinical Examination Findings
Right knee examination reveals moderate joint effusion. Tenderness is noted along the medial joint line. Range of motion is restricted, specifically demonstrating a block to terminal extension (locked knee). McMurray test is positive for a painful click along the medial aspect. Lachman and pivot-shift tests are negative, suggesting intact cruciate ligaments.
Treatment Protocol
Initial management includes RICE protocol (Rest, Ice, Compression, Elevation) and non-weight bearing with crutches. MRI of the right knee is ordered to confirm the bucket-handle tear morphology. Referral to orthopedic surgery is indicated for urgent arthroscopic evaluation and potential meniscal repair or partial meniscectomy.
