Clinical Presentation & Protocol
Patient Usually Complains Of
Patient presents with acute right knee pain following a direct trauma/fall. Reports inability to bear weight and significant swelling. Denies neurovascular deficits, numbness, or tingling in the distal extremity.
Clinical Examination Findings
Right knee inspection reveals significant prepatellar swelling, ecchymosis, and a palpable transverse gap along the patella. Tenderness to palpation over the patellar mid-substance. Active straight leg raise is absent/impaired. Distal pulses (DP/PT) are 2+ and capillary refill is <2 seconds. Sensation intact to light touch in all dermatomes.
Treatment Protocol
Immobilization of the right knee in full extension using a knee immobilizer. Analgesia administered. Radiographic confirmation of transverse patellar fracture obtained. Orthopedic consultation requested for definitive management (ORIF vs. conservative). Non-weight bearing status ordered.
