Clinical Presentation & Protocol

Patient Usually Complains Of

Patient presents with anterior knee pain and localized swelling over the patella. Reports history of repetitive kneeling or direct trauma. Symptoms exacerbated by kneeling or direct pressure. Denies fever, chills, or systemic symptoms. No history of penetrating injury or recent surgical intervention.

Clinical Examination Findings

Physical exam reveals a fluctuant, erythematous, and tender swelling localized to the prepatellar bursa. Skin integrity is intact with no signs of cellulitis or sinus tract formation. Range of motion of the knee joint is preserved, though limited by pain at extremes of flexion. No joint effusion or ligamentous instability noted. Neurovascular status intact distally.

Treatment Protocol

Conservative management initiated: activity modification, avoidance of kneeling, and use of knee pads. Prescribed NSAIDs for pain and inflammation. Advised RICE protocol (Rest, Ice, Compression, Elevation). If refractory, consider aspiration for fluid analysis and corticosteroid injection, or referral for surgical bursectomy.
