Patient Usually Complains Of
Patient presents with chronic anterior knee pain localized to the superior pole of the patella. Symptoms are exacerbated by eccentric loading, jumping, or prolonged sitting. No history of acute trauma or mechanical locking. Pain is described as a dull ache, worsening with activity and improving with rest.
Clinical Examination Findings
Right knee examination reveals localized tenderness at the quadriceps tendon insertion on the superior patellar pole. No significant joint effusion or ligamentous laxity noted. Pain reproduced upon resisted knee extension and during a single-leg squat. Range of motion is full but painful at terminal extension.
Treatment Protocol
Initiate conservative management including activity modification, load management, and a structured eccentric strengthening program for the quadriceps. Consider topical NSAIDs or physical therapy modalities for pain modulation. Monitor for response over 6-8 weeks; consider orthobiologics or imaging if refractory.
