Clinical Presentation & Protocol

Patient Usually Complains Of

Patient presents with right-sided radicular pain originating in the lumbar region and radiating down the posterior/lateral aspect of the right lower extremity. Pain is described as [sharp/burning/electric], exacerbated by prolonged sitting, coughing, or Valsalva maneuver. Associated symptoms include [numbness/paresthesia/weakness] in the right [L4/L5/S1] dermatomal distribution.

Clinical Examination Findings

Right-sided lumbar paraspinal tenderness noted. Straight Leg Raise (SLR) test positive on the right at [degrees] degrees, reproducing radicular symptoms. Neurological exam: Right [L4/L5/S1] motor strength [x/5], diminished right [patellar/Achilles] reflex, and sensory deficit in the right [dermatome]. Gait is [antalgic/normal].

Treatment Protocol

Initiate conservative management including activity modification, physical therapy focusing on nerve gliding and core stabilization, and NSAIDs. Prescribe [gabapentinoid/muscle relaxant] for neuropathic pain control. Consider epidural steroid injection if symptoms persist beyond 6 weeks or if severe functional impairment is present.
