Clinical Presentation & Protocol

Patient Usually Complains Of

Patient presents with chronic, localized pain over the right lateral hip. Pain is described as aching, exacerbated by prolonged standing, walking, and lying on the affected side. No history of acute trauma, radiation of pain down the leg, or neurological deficits. Symptoms are consistent with Greater Trochanteric Pain Syndrome (M70.61).

Clinical Examination Findings

Right hip examination reveals localized tenderness to palpation directly over the greater trochanter. No erythema, warmth, or fluctuance noted. Range of motion (ROM) of the right hip is full and painless in flexion, extension, and rotation, though resisted abduction reproduces the lateral hip pain. FABER and FADIR tests are negative, ruling out intra-articular pathology. Gait is antalgic, favoring the left side.

Treatment Protocol

Conservative management initiated: activity modification, avoidance of sleeping on the right side, and a course of NSAIDs. Referral to physical therapy for hip abductor strengthening and iliotibial band stretching. Consider corticosteroid injection to the trochanteric bursa if symptoms persist despite conservative measures.
