Patient Usually Complains Of
Patient presents with acute onset of right knee pain following a twisting injury. Reports mechanical symptoms including locking, catching, and inability to achieve full knee extension. Denies prior history of instability or trauma to the affected limb. Pain is localized to the medial joint line and exacerbated by weight-bearing and rotational activities.
Clinical Examination Findings
Right knee examination reveals joint line tenderness along the medial aspect. Range of motion is limited by a mechanical block, specifically a deficit in terminal extension (locked knee). McMurray test is positive for a palpable click and pain along the medial joint line. Lachman and pivot-shift tests are negative, indicating intact cruciate ligaments. Effusion is present.
Treatment Protocol
Immediate management includes RICE protocol (Rest, Ice, Compression, Elevation) and non-steroidal anti-inflammatory drugs (NSAIDs). Patient is advised to remain non-weight bearing with crutches. MRI of the right knee is ordered to confirm the bucket-handle tear morphology and assess for associated ligamentous injury. Surgical consultation for arthroscopic meniscal repair or partial meniscectomy is indicated.
