Clinical Presentation & Protocol

Patient Usually Complains Of

Patient presents with a history of progressive dull, aching pain localized to the right lower leg. Symptoms are chronic, exacerbated by weight-bearing, and occasionally associated with nocturnal discomfort. No history of acute trauma, fever, or constitutional symptoms. Patient reports a subjective feeling of bony enlargement or bowing of the right tibia.

Clinical Examination Findings

Musculoskeletal examination of the right lower extremity reveals localized bony enlargement and cortical thickening of the right tibia. Palpation demonstrates increased local warmth and tenderness over the tibial shaft. Gait analysis shows a mild antalgic limp. No evidence of neurovascular compromise in the distal extremity. Range of motion of the knee and ankle joints remains within functional limits.

Treatment Protocol

Initiate pharmacological management with bisphosphonates (e.g., Zoledronic acid or Alendronate) to suppress excessive bone turnover. Administer supplemental Calcium and Vitamin D3 to prevent hypocalcemia. Monitor serum alkaline phosphatase (ALP) levels and bone-specific markers periodically. Orthopedic consultation for bracing or surgical intervention if bowing deformity progresses or fracture risk increases.
