Clinical Presentation & Protocol

Patient Usually Complains Of

Patient presents with acute onset of right knee pain following a high-energy trauma (dashboard injury/hyperextension). Reports immediate posterior knee pain, subjective instability, and difficulty with weight-bearing. No neurovascular deficits reported.

Clinical Examination Findings

Right knee examination reveals significant posterior sag sign. Posterior drawer test is positive (Grade III) with loss of firm endpoint. Lachman and anterior drawer tests are negative. Varus/valgus stress tests are stable. Neurovascular status is intact distally.

Treatment Protocol

Initiate RICE protocol (Rest, Ice, Compression, Elevation). Immobilization in a hinged knee brace locked in full extension for 4-6 weeks. Non-weight bearing or toe-touch weight bearing as tolerated with crutches. Physical therapy referral for quadriceps strengthening. Surgical consultation for potential reconstruction.
