Patient Usually Complains Of
Patient presents with acute right knee injury following a valgus stress mechanism with rotational force. Reports immediate "pop," significant pain, and rapid onset of hemarthrosis. Patient describes a sensation of knee instability, locking, and inability to bear weight.
Clinical Examination Findings
Right knee examination reveals significant joint effusion. Lachman and Anterior Drawer tests are positive, indicating ACL rupture. Valgus stress test at 0 and 30 degrees is positive, indicating MCL injury. Joint line tenderness is present, with positive McMurrayโs test, suggestive of medial meniscus tear. Range of motion is limited by pain and mechanical block.
Treatment Protocol
Immediate management includes RICE protocol (Rest, Ice, Compression, Elevation), knee immobilization with a hinged brace, and non-weight bearing status with crutches. Referral to orthopedic surgery for MRI evaluation and surgical planning for ACL reconstruction, MCL repair/reconstruction, and meniscal management.
