Clinical Presentation & Protocol
Patient Usually Complains Of
Patient presents with acute onset of right knee pain and mechanical locking following a twisting injury. Reports inability to fully extend the knee, associated with localized lateral joint line tenderness, swelling, and a sensation of "giving way." No history of prior instability or chronic degenerative symptoms.
Clinical Examination Findings
Right knee examination reveals significant joint effusion and restricted range of motion, specifically a block to terminal extension. Palpation demonstrates exquisite tenderness along the lateral joint line. McMurrayโs test is positive for a palpable click and pain on the lateral aspect. Lachman and pivot-shift tests are negative, indicating intact cruciate ligaments.
Treatment Protocol
Immediate management includes RICE protocol (Rest, Ice, Compression, Elevation) and non-weight bearing status with crutches. Surgical intervention via arthroscopic partial meniscectomy or meniscal repair is indicated to address the bucket-handle displacement and restore mechanical function. Pre-operative MRI confirmed the diagnosis.
